神經系統長新冠在香港

香港大學李嘉誠醫學院 - 臨床醫學學院內科學系腦神經科/ 瑪麗醫院，誠邀 閣下參與有關神經系統長新冠的網上問卷調查，詳情如下：

我們邀請所有十八歲或以上曾經感染2019新型冠狀病毒的香港居民參與一項2分鐘的網上問卷調查。完成是次問卷調查的人仕可以參加抽獎，有機會獲得港幣$300超市現金券(共20個名額)。參與此項網上問卷調查純屬自願性質。所有經此研究收集的個人資料不會被泄露給第三方，並會嚴正保密。

The Division of Neurology, Department of Medicine, School of Clinical Medicine, Li Ka Shing Faculty of Medicine, The University of Hong Kong/ Queen Mary Hospital, we sincerely invite you to participate in the online survey on ‘Neuro-Long COVID in Hong Kong’. Details are as follows:

Hong Kong residents aged 18 or above who have been infected with COVID-19 virus are cordially invited to complete a 2-minute online survey. Upon valid completion of this survey, participants will be eligible for a lucky draw with a chance of winning HKD$300 supermarket voucher (a total of 20 quota). It is at your own discretion to participate in this online survey. All personal information will not be disclosed to any third parties and will be kept strictly confidential.
